Warning: The use of the Website by individuals under 18 years of age is strictly prohibited.

We process data of individuals under 18 years of age only with the consent of their legal representative, to ensure the proper provision of services contracted by the legal representatives of minors. If an event leads to the processing of personal data of children under 18 in situations other than those mentioned in this paragraph, we will immediately cease processing upon discovering such an event.

It is possible that we receive information about individuals under 18 due to fraud or deception by a third party. 5.1 Accessing the Website

Processed personal data:

When you, as a Visitor, interact with the Website (access/visit it), certain information is automatically transferred between your device and our server, which automatically records the transferred information.

5.2.1. Via email

Personal data processed:

When you send a message to the email address associated with Customer Service, as displayed on the Website, we collect your email address and any other information you provide in that email, such as your name, phone number, and the information contained in any email signature block.

5.2.2. Via the contact form available on the Website

Personal data processed:

When you contact us using the contact form, we collect your first name, last name, email address, and phone number. Additionally, we collect any other information you provide when filling out the contact form in the message field.

If you do not provide the required information in the contact form, you will not be able to submit it, and we will not receive your request. Consequently, we will not be able to respond.

When you contact us by phone, we collect your phone number and any information you provide during the conversation.

The medical office records telephone calls made by you. Thus, we may process your voice (call recordings) only with your prior consent.

If you do not consent to the processing of your voice, we will not be able to continue the phone call, and as a result, we will not be able to answer your questions.

5.2.4. Via postal mail

When you contact us by mail, we will collect your first and last name, postal address, and/or any other information contained in the postal communication you send.

If you do not provide your postal address and identification details, although we will receive your request, we will not be able to respond to it.

Visitors to the Website have two options for scheduling an appointment for one of the services offered by the Medical Office: by phone, using the contact number, or through the online appointment form available on the Website. In order to schedule an appointment for medical services, we require the Patient’s data (your data or the data of the person for whom you are making the appointment, for example – a minor).

5.3.1. Through the Website – Online Appointment Form

When you contact us by phone to make an appointment, we collect: name, surname, age, phone number and your phone number and any additional information you provide regarding the appointment.

The medical office records the telephone calls made by you. So that we can also process your voice (telephone conversation recording), only with your prior consent.

If you do not express your agreement regarding the processing of your voice, we will not be able to continue the telephone call and consequently, we will not be able to answer your questions.

If you make the appointment for another person, for example – a minor, we will collect your contact details and the minor’s identification data (as a patient), as well as any other information regarding the appointment, as provided by you, directly and voluntarily.

5.4. Registration on the Website

If you do not provide all the mandatory information required by the registration form, you will not be able to register or create an account on the Website and you will not benefit from the functionalities associated with the account.

However, the lack of creating an account does not in any way affect your ability to browse the Website and access the services provided through it.

  • Data managed in the account: appointment history (details of previous appointments), medical history (investigations, interventions, previous treatments);
  • Interaction of the user with communications received from the Medical Practice;
  • Other data: reviews of the medical services the user has received at the Medical Practice.

    The medical office will process personal data for the duration necessary to fulfill the purposes specified in this Policy, and thereafter, in order to comply with applicable legal obligations.

As a general principle, the medical office will process personal data throughout the duration of the contractual relationship. Afterward, personal data will be stored for the periods applicable to various types of personal data, as determined in accordance with applicable legal provisions, such as the retention period for medical records (in accordance with the legislation regarding the archiving of medical files) or financial-accounting documents, for which the retention period specified by law is 5 years, calculated from July 1 of the year following the one in which the financial year in which they were issued ended.

If the processing is based on your consent, in case you withdraw your consent for data processing and there are no other legal grounds for processing (e.g., a legal obligation, a contractual relationship, or the legitimate interest of the medical office), or you object to the processing and there are no overriding legitimate grounds for processing, the medical office will cease processing your personal data.

After the storage period mentioned above expires, the medical office may anonymize this data, thereby removing its personal character, and continue processing the anonymized data for statistical purposes.

    We will not transfer your personal data outside the European Economic Area.
